Are you a Registered Nurse with 3 or more years experience in long term care? As a Nurse Case Manager at Miller’s, you are responsible for the nursing care and service to the patients.  You plan and oversee the Comprehensive Care Plan for your patients and are responsible for thorough and accurate documentation in the patient record.  You oversee care on your unit that will comply with company policy and the State and Federal regulations governing those services.

This position will also include Admissions and Medical Records Duties



1. Currently licensed and registered to practice as a professional nurse in the State of Indiana.

2. Successfully complete pre-employment process and screening.

3. Pass the physical examination given in behalf of gainful employment within the facility.

4. Professional manner and appearance.



1.  Assist with pre-admission assessments as requested

2.  Ensure rooms are ready for new admits with any special equipment needed set up prior to arrival

3.  Assist with chart preparation and new admission orders

4.  Welcome new admits to facility upon arrival or as soon as possible and assist with adjusting to room and reviewing of facility norms

5.  Complete 450B's within 48 hours of admission

6.  Attend and lead initial Health Care Conferences and all subsequent health care plan conferences

7.  Coordinate, attend and lead all Discharge Planning meetings.  Discuss plans for discharge, level of care, needs and options.  Coordinate discharge planning, resident and family education and completion of the "discharge planning and teaching tool" and PASSPORT.  Obtain discharge orders and make home health care referral if appropriate

8.  Have frequent communication with the Therapy Department to maintain current information on residents' status.  Attend weekly Medicare meetings

9.  Review all charting and new physician orders daily (when scheuled).  Follow up as necessary

10.  Ensure all physician orders are followed

11.  Ensure care plans are initiated at time of admission and updated with changes/new orders

12.  Ensure physician orders are up to date in PCC

13.  Ensure CNA assignment sheets are updated as needed

14.  Meet with all residents daily (when scheduled) to assist as needed and follow up on changes/concerns

15.  Obtain physician order for discharge when appropriate and coordinate including, but not limited to, teaching, medications, equipment and outside services required

16.  Complete wound rounds weekly, on same day of week, and document timely

17.  Complete Medicare non-coverage letters as required

18.  Complete QA calls and tools as assigned

19.  Be able to communicate upcoming discharges upon request

20.  Complete post discharge calls per policy

21. Any other duties as assigned by supervisor
